Vendor declaration

AFIA has developed a Vendor Declaration Form for use by members. The Form is now available in an electronic format (PFF file) and the standard PDF format. Both can be downloaded to your PC. You will need to download the Formatta Filler to use the PFF format (about 1Mb) and Adobe Acrobat to use the PDF format. Both downloads are free.

Using the PFF format allows you to fill the form out on the screen, save copies on your PC, email the form from a button on the form and print out the form with contents included. Standard data can be saved into a master copy that becomes the base Vendor Declaration that you work with.

Please note that if you use Netscape 6 or Netscape Communicator as your browser, you will not be able to open and display the PFF file within your browser. You will have to save it to your hard disk.
